Below is important information about insurance, payment options, financial responsibilities, and how to apply for our Sliding Fee Discount Program.
Insurances We Accept
BRHC accepts most major insurance plans, including:

- Marketplace/ACA plans
- Behavioral health and dental insurance plans

If You Are Uninsured
If you don’t have insurance, you can still receive care at BRHC. You may qualify for:

Your Financial Responsibilities

BRHC offers a Sliding Fee Discount Program to ensure care remains affordable regardless of income or insurance status. Discounts are based on:

- Federal Poverty Guidelines
You may qualify for reduced costs on:

- Behavioral health services
- Dental services
- Laboratory services
- Select medications through referral partners
How to Apply
To apply, please bring:
- Proof of income (paystub, tax return, benefits letter, etc.)

- Photo identification

Good Faith Estimates/GFE (No Surprise Act)
You have the right to receive a Good Faith Estimate explaining expected costs for medical or dental services if you:
- Do not have insurance, or
- Do not plan to use insurance
To request an estimate: Ask for a GFE when scheduling an appointment.

Yes, we offer a 10% prompt pay discount if you pay your bill in full at the time of service or 20 days from your first statement.
Do you offer Good Faith Estimates?
Yes. Uninsured or self‑pay patients may request an estimate before receiving services.
